The RN Care Manager provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The RN Care Manager demonstrates leadership by serving as an advocate for patients, team player to colleagues as he/she continues to enhance his/her own and the team's professional growth, development, and practice. The RN Care Manager promotes access with non-face-to-face visits, when appropriate, through answering patient calls/triage messages and performs scheduled clinic visits, walk-in or urgent visits, and telephone visits. Make arrangements for timely appointments, follow ups, and initiates recommendations made by the provider. Triages phone calls and responds to caller's needs. Speaks with and handles requests from patients, family members, clinicians, nurses, outside facilities, and internal customers. Ensures the transfer of medical information, imaging, and pertinent records for lnterfacility consults (IFCs) and other outside agencies that are providing medical care for surgical specialty patients occurs in a timely manner. Tracks care of patients and follows up to promote compliance with medical care, identify any issues creating challenges for the patient population and analyzing the process and offering creative solutions. Collects, maintains, and analyzes workload data for their specialty area(s). Responsible for acquiring and demonstrating clinical expertise and knowledge in their care area(s) to improve the quality of patient care, the use of resources, and to facilitate compliance with internal and external requirements and standards. Technical functions include electrocardiography (EKG), ballistocardiograph (BCG) administration urinary catheterizations, and wound dressings. Communicates and collaborates with IDT to develop and coordinate the treatment plan for patients in the outpatient setting. The incumbent will maintain certification in Basic Life Support (BLS). Demonstrates ability to function effectively in charge nurse, team leader or other leadership roles requiring application of problem skills at the unit or team level and uses team approaches to identify, analyze, and resolve problems. Ensures adherence to policies and procedures, i.e., Bar Code Medication Administration (BCMA), documentation, and Accucheck. Serves as a role model in assessing, planning, and evaluating patient status and identifying priority needs for care throughout the health continuum. Develops and updates the nursing care plan, serving as a resource person to less experienced staff. Includes patient/family/significant other/health care providers in establishing realistic, measurable goals related to the treatment plan and health education. Demonstrates skill in implementation of nursing and physician orders for patients with complex patient care needs. Assesses and treats pain utilizing the pain management protocol. Documents administrative and clinical information in a thorough and accurate manner. Meets requirements for age related competencies for the assigned unit:
For young adulthood (18-40), accepts chosen lifestyle, assists with necessary adjustments relating to health and recognizes the person's commitments in his life. For middle adulthood (41-64), assists patients to recognize the risk factors related to health, to plan for anticipated changes in life, and to focus on strengths rather than weaknesses. For older adulthood (65-84), assists patients to keep physically active within health limitations, socially active by maintaining peer group interactions, and assist patients to cope with loss and provide for necessary safety measures. Demonstrates the ability to assess data reflective of the patient's status and care requirements for population-specific needs, such as, culture, Veteran era, etc., and plans and delivers care appropriately. Patient identifiers are used to reliably identify patients. Employee uses acceptable identifiers per facility policy Blood and other specimen containers are labeled in the presence of patient. Compliant with hand hygiene. Complete list of medications is provided to the patient on discharge or when leaving the facility from an outpatient appointment, or after a home visit. Patients are assessed for all risk. For patients at risk, fall prevention interventions and effectiveness are documented. Initial pressure ulcer risk assessment done at admission using valid tool; and ongoing per policy; interventions documented for at risk patients. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Standing (up to 2 hours), walking (up to 6 hours), stooping, bending, reaching, lifting, and turning; repositioning patients, assisting with transfers; pushing or lifting more than 45 pounds; visual (corrected to 20/30 or better with color discrimination); auditory acuity; arms above shoulder level; neck in a fully flexed or extended position; work on computers for extended periods of time. Work is primarily conducted in a well-ventilated, adequately heated and cooled building. May be exposed to inclement weather when traveling around medical center. May be exposed to normal office irritants - dust, pollen, noise, vibrations, or odors. May be exposed to therapy animals with and without advanced notice. May be exposed to patients and family in emotional upheaval or extreme pain. Must have emotional stability to properly care for usual patient population.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
