We are seeking a Social Worker with an exceptional amount of customer service, compassion and dedication to be part of an incredible care team.
We are currently hiring for:
Part Time, 20 hours Biweekly, Monday Through Friday What you get:

Valid PA Driver's License.
Bachelor's degree in social work is required.
What you do:
Obtains social and medication history, financial information, and other relevant information required for the admission process.
Provides information and explains the admission policy, nursing home programs and costs, Medicare and Medicaid determinations to prospective residents, families and physicians.
Develops and maintains continuing contact with residents to assist with the adjustment to the nursing home environment and attends meetings regarding progress.
Assists residents and their families with discharge/post-discharge plans and applying for medical assistance.
Some of these listed functions may be performed in coordination with or in the absence of an admissions coordinator/liaison as directed by the nursing home administrator.
Participates in interdisciplinary team resident care planning and other interdisciplinary committees as assigned.
Prepares social work notes and recommendations to be included in resident's medical records following Luthercare's medical record documentation standard.
Adheres to all Healthcare Center standards and regulations; such as but not limited to infection control, fall prevention, general safety protocols and quality assurance and performance improvement standards.
Monitors hospitalization of residents with family members or appropriate hospital personnel for any change in placement status.
Attends utilization review meetings.
Provides referrals for specialized services as appropriate.
Provides residents with appropriate and timely regulatory notices including but not limited to insurance coverage notices of coverage/non-coverage as required.
